Nightmare is only halfway through the difficulty chain. Melee are reportedly the most difficult classes to play in Inferno. There are several posts about the barbarian needing a buff for the higher difficulties. Possibly noob threads I have no clue but I myself have noticed that when shit gets hairy my ranged classes can just haul ass while I have to use abilities to get out of combat as melee (especially barbarian).
I will admit that I go for a dps build instead of a vitality build but it looks like vitality isn't too important until hell/inferno anyways.

Yeeeah, Barbarian in Hell has been... Hell. I die constantly. Normal was just click-through-entire-game, watch-cutscenes. Now I'm dying 3-5 times a fight on elites. asldkjasdlkfjasdfljk BUT I WILL PERSEVERE. My Demon Hunter has actually been having even less trouble, but she's still in Normal.

I recommend this build for a Barbarian in Hell and beyond, so far it has worked wonderfully.
